Insurance
Our Practice participates in many insurance plans including VSP, and Eyemed, and also many medical plans including Medicare, Blue Cross Blue Shield, United Health Care, and Aetna. Please call us for a full list of insurance options.
If you are covered by a medical plan or vision plan, it is your responsibility to know your benefits and eligibility. We will attempt to confirm your eligibility, but if it is determined that you are not covered, payment in full is expected at the time of visit or when the patient is billed by our office. If your insurance changes, please let us know at your earliest convenience so we can update your information and apply insurance to your visit.

- Copayments and Deductible: These are payable at time of services.
- Non-Covered Services: Your vision or medical plan may not cover all services that Dr. Plank determines is necessary at the time of visit. If we expect that your plan may not cover a procedure or service, we will notify you in writing prior to performing the procedure or service. Medicare patients will be notified via an ABN, and for commercial patients with a Notice of Exclusion from Health Benefit Plank (NEHBP). These documents will list the recommended procedures as well as the expected out of pocket costs to you so that you can make an informed decision before proceeding.
